2009-2010 Sample IEPs with Completed Transition Sections

We have created some sample IEP forms using the new Ohio IEP forms. We only completed the sections of the IEP pertaining to Transition (sections 1, 4, 5, 6 and one part of 7) in our samples.
PLEASE READ THIS DISCLAIMER:
The samples represented on this website are intended to assist teachers complete the transition section of the IEP. These specific samples have not been approved by the Ohio Dept of Education/Office of Exceptional Children. The ODE/OEC will not provide samples because they do not want teachers to simply "cut and paste" from samples onto an individual student's IEP form.
We trust that the professional educators with whom we work will use these samples for the purpose for which they were intended--as a GUIDE to help maneuver through a complicated and new process.
How to use the samples:
The name of the "student" will give you a clue as what type of student we had in mind when we wrote the sample. For example, "Jimmy Junior Carpenter" will be an 11th grade student whose postsecondary goal is to become a carpenter. "Freddy Sophomore University" will be a 10th grader whose postsecondary goal is to go to a 4 year college.
We will add more samples as we create them.
