Examples of Age Appropriate Transition Assessments

 

  • Document the assessments you used  and the date completed in Section 4 of IEP form in box entitled " Age Appropriate Transition Assessments"
  • Do not reinvent the wheel—use assessments that are already in place.(see list below)
  • Assessments may be formal or informal.

 

 

Look at Student’s most recent ETR:

 

            Adaptive Behavior Scales

            Social Emotional Scales

            Vocational/Occupational Assessment

            Achievement Scores

            General Intelligence Tests

            Etc

 

Student's Trancript or report card

 

ACT test –includes interest inventory component

 

SAT test-- includes interest inventory component

 

PLAN test-- includes interest inventory component
