Examples of Age Appropriate Transition Assessments

Document the assessments you used and the date completed in Section 4 of IEP form in box entitled " Age Appropriate Transition Assessments"

Do not reinvent the wheel—use assessments that are already in place.(see list below)

Assessments may be formal or informal.

Look at Student’s most recent ETR:
